Patient Advocacy &
Care Coordination Services

Personalized Healthcare Navigation for Individuals, Seniors & Families

Healthcare can become overwhelming when you're managing multiple doctors, appointments, treatment decisions, an aging parent's care, insurance issues, or a transition to a new level of care.

The Patient Advocate Agency provides private, concierge-level patient advocacy and care coordination to help individuals and families understand their options, organize care, communicate with providers, and manage the many details of a complex healthcare journey.

Explore our five primary service areas below to learn how we can help.

Provider Coordination • Appointments • Hospital Discharge • Care Transitions

Navigating healthcare can be overwhelming—especially when you're managing multiple doctors, specialists, medications, diagnoses, appointments, and follow-up care.

Our Care Coordination and Case Management services provide the structure, communication, and support needed to help keep your healthcare organized, connected, and moving forward.

We serve as your personal healthcare advocate and care coordinator, helping communicate among doctors, specialists, therapists, hospitals, and care facilities involved in your care. We coordinate appointments, organize medical information, track important follow-up, help you understand treatment plans, and work to make sure important details don't fall through the cracks.

For clients leaving a hospital or skilled nursing facility, we help coordinate the transition to home or another level of care. This may include reviewing discharge plans, coordinating follow-up appointments, helping arrange equipment and services, communicating with providers, and making sure the patient and family understand what comes next.

For seniors and aging parents, we can also help families coordinate multiple providers and changing healthcare needs. For families who live outside the Palm Springs area, we can provide local advocacy and care coordination to help keep designated family members informed and involved.

Our goal is to help reduce gaps in care, improve communication among everyone involved, and give patients and families greater clarity and confidence throughout the healthcare journey.

This service is especially helpful for:

  • Individuals with chronic or complex medical conditions

  • Seniors managing multiple doctors and specialists

  • Families coordinating care for an aging parent or spouse

  • Long-distance family members managing a loved one's healthcare

  • Clients transitioning from a hospital or rehabilitation facility

  • Anyone overwhelmed by appointments, medical instructions, records, or next steps

Board & Care • Assisted Living • Memory Care • Skilled Nursing • Care Transitions

Choosing the right care and living arrangement for an aging parent or spouse can be one of the most difficult decisions a family faces. As healthcare and personal-care needs change, families may need help determining whether their loved one can safely remain at home or whether Board & Care, Assisted Living, Memory Care, or Skilled Nursing may be appropriate.

The Patient Advocate Agency helps families understand their options, evaluate care needs, research appropriate facilities, and coordinate transitions. We can review licensing and available quality information, discuss staffing and care considerations, compare services and costs, and help families ask the right questions before making a decision.

Our involvement doesn't have to end once a facility is selected.

For clients in Palm Springs and throughout the Coachella Valley, we can provide ongoing advocacy and care coordination, communicate with facility staff and healthcare providers, participate in care meetings when appropriate, review changes in care, and keep designated family members informed.

This can be particularly valuable for adult children who live outside the area and need a trusted local advocate to help oversee an aging parent's healthcare and changing care needs.

This service is especially helpful for:

  • Families caring for an aging parent or spouse

  • Seniors considering whether to remain at home or move to a higher level of care

  • Families evaluating Board & Care, Assisted Living, Memory Care or Skilled Nursing

  • Seniors transitioning from a hospital or rehabilitation facility

  • Long-distance families needing local advocacy and oversight

  • Families who want independent guidance when making senior-care decisions

Treatment Research • Second Opinions • Advanced Therapies • Clinical Trials • Specialized Care

When you or someone you love is facing a serious diagnosis, complex medical condition, or uncertainty about a current treatment plan, understanding all available options can be difficult. Patients are often limited to the specialists, treatments, and resources available within their immediate healthcare system.

The Patient Advocate Agency helps clients research treatment options, specialists, medical centers, second opinions, advanced therapies, clinical trials, and other appropriate healthcare resources locally and nationally. For complex or less common conditions, we can broaden the search beyond the Coachella Valley to identify physicians and medical centers with relevant expertise.

We help organize the information so clients and families can better understand the options available to them and prepare for productive conversations with their treating physicians. When additional expertise is needed, we can also help coordinate medical records, consultations, second opinions, and appointments.

Our role is not to diagnose a condition or recommend a particular medical treatment. We provide research, organization, and advocacy so clients can ask informed questions and make healthcare decisions in consultation with their licensed medical providers.

This service is especially helpful for:

  • Individuals facing a new or serious diagnosis

  • Patients who want to explore additional treatment options

  • People seeking a second opinion

  • Patients with complex, uncommon, or difficult-to-treat conditions

  • Families looking for specialized physicians or medical centers

  • Patients interested in learning about clinical trials or emerging treatment options

  • Individuals who feel they have exhausted the options available locally

Medical Billing • Insurance Claims • Pre-Authorizations • Insurance Appeals

Medical bills and insurance issues can be confusing and stressful, especially when you're already managing a serious illness, complex healthcare needs, or caring for a loved one. The Patient Advocate Agency helps clients understand their medical bills and insurance coverage, identify potential billing errors, and work through complicated claims and coverage issues.

We assist with insurance claims, pre-authorizations, denied claims, and insurance appeals. When a claim or authorization is denied, we help clients understand the reason, organize the necessary medical records and supporting documentation, and communicate with insurance companies, healthcare providers, and billing departments as appropriate.

For medical billing concerns, we review bills and statements for potential errors, duplicate charges, coding or coverage issues, and other discrepancies.

When appropriate, we communicate with providers and billing departments to seek clarification and help resolve disputed charges.

Medical billing and insurance advocacy may be provided as a stand-alone service or as part of a larger care-coordination relationship when insurance, authorization, or billing problems interfere with accessing or coordinating care.

This service is especially helpful for:

  • Clients with unexpected or confusing medical bills

  • Individuals dealing with insurance claim denials

  • Patients needing pre-authorizations for treatments, procedures, or medications

  • Families managing complicated insurance paperwork

  • Clients who believe a medical bill may contain an error

  • Patients having difficulty obtaining insurance approval for needed care

Physician Research • Specialists • Second Opinions • Local & National Resources

Finding the right doctor or specialist can be difficult, particularly when symptoms aren't improving, a diagnosis is complex, or the expertise you need isn't readily available in your local area.

The Patient Advocate Agency helps clients research physicians and specialists based on their healthcare needs, diagnosis, location, insurance coverage, and other important considerations.

We can search locally in Palm Springs and Southern California or expand the search nationally when specialized expertise or another opinion is needed.

Our research may include physicians and specialists in areas such as cardiology, neurology, oncology, rheumatology, orthopedics, mental health, and complex or less common medical conditions.

We help clients compare potential providers, understand their areas of expertise, and identify appropriate options to discuss with their existing healthcare team.

Once a physician or specialist has been identified, we can help coordinate referrals, medical records, imaging, test results, appointments, and insurance requirements. We can also assist with coordinating second opinions when clients want another perspective on a diagnosis or treatment plan.

Our goal is to make finding and accessing appropriate medical expertise easier, while helping patients and families stay organized and informed throughout the process.

This service is especially helpful for:

  • Patients seeking a specialist or second opinion

  • Individuals with complex or unresolved healthcare concerns

  • Clients looking for expertise that may not be available locally

  • Families researching physicians or medical centers for a loved one

  • Patients who need help coordinating records and referrals

  • Individuals looking for in-network or out-of-area specialists
